Abstract:This study was done to evaluate the association between iron level in the blood and in the liver tissue in different chronic hepatic diseases other than hemochromatosis and to establish if there is an etiologically based relaThis study included 60 patients,They were classified into two groups.Group I suffering from chronic hepatitis C +/- liver cirrhosis and Group II with hepatocellular carcinoma on top of cirrhotic liver.It was found that there was a positive correlation between grade of histological activity of chronic hepatitis,transferrin saturation,TIBC and iron overload status within the liver tissue in chronic hepatitis group.There was a negative correlation between grade of differentiation of hepatocellular carcinoma,TIBC and iron overload status within the liver tissue in hepatocellular carcinoma group.

Abstract:Hepatitis C infection is an emerging epidemic.Hepatitis C virus is a major cause of morbidity and mortality in Egypt.The mechanisms leading to liver cell injury,inflammation and fibrosis in chronic hepatitis C,are not fully understood.Apoptosis have been postulated as a mechanism of liver cell injury in chronic hepatitis C.Apoptosis is a form of cellular death in which the cell breaks up into several apoptotic bodies that are phagocytosed by the phagocytic system without any evidence of inflammatory reaction.Fas antigen is an important mediator of apoptosis.Overexpression of P53 has been extensively studied in HCC,but fewer observations have been made in non-tumoral liver diseases.The aim of this study was to detect the role of apoptosis as a mechanism of liver cell injury in chronic hepatitis C and to correlate it with the degree of liver cell injury.In this study,the immuno histochemical technique was used to localize the extent of expression of both Fas antigen and P53 in liver tissues with chronic hepatitis C of various grades of inflammatory activity.One hundred and twenty seven patients with clinical and/or laboratory evidence of chronic liver disease referred to Tropical Medicine Department,Theodor Bilharz Research Institute over a period of two years,were surveyed for the presence of HCV infection were subjected to full clinical,laboratory,abdominal ultrasonographic and endoscopic examinations.57 patients selected for this study having chronic hepatitis C virus infection were subjected to PCR for detection of viraemia levels and liver biopsy for histopathological assessment of the degree of liver cell injury and of the stage of liver fibrosis.Five healthy individuals were included in this study to serve as controls.Liver tissues of both patient and control groups had been examined by immunohistochemical techniques (detection of both Fas expression for apoptosis and p53 expression for liver cell proliferation)for detection of apoptosis.Electron microscopic study of liver tissues of 8 patients done to study the detailed ultrastructural features of liver cell apoptosis.The current study showed Fas positivity in 84.2of the patient group pointing to the prevalence of the apoptotic process in chronic hepatitis C.Significant direct correlations were detected between grading of inflammation,staging of fibrosis,serum ALT,AST,ALP and HCV-RNA levels relative to Fas positivity in HCV-infected patients.In the present study,p53 nuclear accumulation was detected in only 29.8of the studied HCV cases.Significant direct correlations were detected between grading of inflammation,staging of fibrosis,serum bilirubin and albumin,ALP and HCV-RNA viraemia levels relative to p53 positivity in chronic HCV patients.However,no significant correlation was detected between either serum ALT level or serum AST level relative to p53 positivity in our study group.High significant direct correlation were detected between p53 positivity and Fas positivity.Significant direct correlations were detected between hepatic steatosis and both Fas positivity (p0.05)and p53 positivity (p0.01). The extent of apoptosis (Fas antigen expression)could point to the degree of activity in chronic viral C hepatitis.P53 is over expressed in the precocious stages of HCV-related liver damage before carcinogenesis.

Abstract:Since the relationship between host factors and response to interferon (IFN)therapy is still a big basal.This study was conducted to : (i)detemine the influence of pegylated IFN vs standard IFN on these cytokines,and (ii)establish the possible relationship between these cytokines and the outcome of treatment.Thirty eight patients were investigated for the serum level of previously mentioned cytokines in a randomized,opened and controlled trial (22 patients treated with pegylated IFN and 16 patients treated with standard IFN). Serum cytokines levels were measured at 0,1 and 12 months of IFN therapy by (ELISA). Our results indicated that serum levels of TNF-RII,IL-8,sFas,and sFas-L have the potential to be used as non- invasive serological markers for the long- term response to pegylated IFN therapy.Moreover,TNF-RI

 RII have the potential to be used for standard IFN treatment.Persistent correlations between sFas and TNF-RII were noted,which may elaborate the possible role clearance with pegylated IFN treatment.

Abstract:Radiofrequency ablation (RFA)and percutaneous ethanol injection (PEI)therapy are currently used for the treatment of hepatocellular carcinoma (HCC). The purpose of the study was to evaluate the usefulness of combination therapy of PEI and RFA (PEI-RFA). Twenty-three cirrhotic patients with 36 hepatic focal lesions which proved to be malignant by Fine Needle Aspiration Cytology (FNAC)or Fine Needle Aspiration Biopsy (FNAB)or Spiral C.T.were enrolled in the study.They were divided into two groups:group I,12 patients (11 males and 1 female)with 21 HCCs (only 12 of them were treated)using RFA alone.Group II,11 patients (9 males and 2 females)with 15 HCCs (only 12 of them were treated)using RFA after PEI.The other focal lesions in both groups were included and managed in other studies.The volume of coagulated necrosis and tumor ablation was estimated and compared in both groups.It was found that the number of ablated tumors in group II was significantly higher than that in group I (100Vs 58.33)with fewer treatment sessions required for complete tumor ablation.No major complications occurred in both groups.Results indicated that PEI-RFA is more effective than RFA alone and can make dramatic improvement of therapeutic effects in RFA therapy for HCC with fewer sessions of treatment.

Abstract:This study was done to evaluate hepatic and splenic arterial blood flow and arterial resistance indices by duplex color Doppler ultrasonography in patients with different grades of liver cirrhosis and portal hypertension.This study included 60 patients who were suffering from portal hypertension due to liver cirrhosis and had oesophageal varices.They were classified into three groups according to Child Pugh's classification.A fourth group consisted of 20 control subjects were used as a reference range for Doppler parameters.Hepatic arterial resistance indices increase in cirrhosis.The correlation between hepatic arterial resistance and the severity of portal hypertension suggests that the mechanism responsible for the increase arterial resistance is parallel to that responsible for the increase in portal resistance.Splenic artery resistance indices are increased in cirrhotic patients and they are not correlated with the severity of portalhypertension but correlated with the degree of oesophageal varices.

Abstract:Background and aim of work:Colorectal cancer (CRC)is a major cause of cancer mortality and morbidity worldwide yet it is considered a preventable disease.Screening asymptomatic persons by fecal occult blood test (FOBT)can reduce mortality from CRC.This prospective study was designed to assess the value of FOBT screening in the Egyptian population.Patients and methods:Patients were recruited as follows:Group 1 included 42 asymptomatic individuals who participated in a screening program of FOBT and colonoscopy while the other groups included 70 patients referred for colonoscopy.Patients were subjected to colonoscopy and findings were correlated with two different types of FOBT:Guaiac-based and immunochemical tests.Results:31 cases of CRC and 22 cases with polyps were detected.33 patients had normal colonoscopic findings.iFOBT was positive in 45 cases and negative in 55 cases with a sensitivity of 69and a specificity of 86while gFOBT was positive in 58 cases and negative in 42 cases with a sensitivity of 78and a specificity of 82for all lesions.gFOBT had a sensitivity of 93for CRC and 45for adenomatous polyps while iFOBT was 87and 36respectively.Conclusions:FOBT provides an acceptable screening method for CRC.iFOBT is a more specific test for screening than gFOBT but has a lower sensitivity in that respect.

Abstract:Chronic colonic diseases are divided into Inflammatory Bowel Diseases (IBD)which include Ulcerative Colitis (UC)and Crohn's Disease (CD), colorectal cancer and colonic polyposis -13.5 syndromes which include familial Adenomatous polyposis (FAP)and Peutz Jegher syndrome.The risk for Ulcerative Colitis associated Colorectal Cancer is increased at least 2 folds compared to the normal population.Colorectal cancer is observed in 5.5 -13.5of all patients with Ulcerative colitis and 0.4- 0.8- 0.8 of patients with Crohn's disease.Colorectal cancer remains the second leading cause of cancer death in USA.It is considered to be the fourth most common malignant neoplasm representing 6.1 of cancers in Egypt The search for IBD-susceptibility genes has resulted in the identification of several loci with potential relevance to IBD etiology.These include IBD1 (MIM 266600), a putative CD-susceptibility locus that maps to the pericentromeric region of chromosome 16 and IBD2 (MIM 602458), mapped to a 41-cM region,on chromosome 12,surrounding the D12S83 marker and implicated in both CD and UC The identification of 5q31-33 and 19p13 as potential locations for IBD-susceptibility genes is of particular interest in view of the biology of the genes known to map within these regions.One of the genes causing susceptibility to CD was recently identified as the CARD15/NOD2 gene The etiology of colorectal cancer is unknown but appears to be multifactorial in origin.The relative risk of developing colorectal cancer is increased in the first-degree relatives of affected patients.The defect is in the adenomatous polyps coli (APC)gene,which is located on chromosome 5 at locus q21.Defective mismatch repair (MMR)genes located on chromosomes 2,3,and 7.PTEN is a candidate tumor suppressor gene that has been isolated and localized at chromosomal band 10q23 The high hope of genetic medicine for 30 years has been to develop a way of using recombinant DNA techniques to treat patients through the genes involved in their diseases Examples for gene therapy:-I)Immune Stimulation :- A)Utilization of human leukocyte antigen (HLA)to stimulate T-cell response.B)Utilization of cytokines to stimulate T cell response.C)Vaccination against tumour-specific antigens.II)Mutant gene correction III)Virus-directed enzyme prodrug therapy.IV)Oncolytic virus therapy.V)Inhibitors of angiogenic growth-factors and their receptors Gene therapy for colon cancer is still at an early stage of development.Preclinical studies have prompted several phase I trials.As our understanding of the molecular biology of cancer increases,gene therapy is likely to have an increasingly important role in the expanding array of treatment options for colon cancer.

Abstract:Objectives:The objective of this study was to detect the seroprevalence of anti-HAV antibodies in the middle socio-economic class (SES), medical students,Kasr El Aini hospital,Cairo University,Egypt.Methods:BioELISA test was done to detect HAV antibodies,using sera of 182 volunteered medical students (preclinical,clinical)and house officers of the middle SES,aging 17-30 years old.Results:The serosurvey revealed that 19.2 of the studied subjects were seronegative;with the age 17-20 years old and the preclinical group of students were the most prone to infection with HAV.Analysis of data obtained from questionnaire about the risks of exposure to the HAV infection revealed significant statistical difference as regards the frequency and the level of place of outdoor food intake and the history of food borne diseases;while no significant statistical difference was detected as regards:Sex,SES (father's occupation,residence), sources of drinking water,exposure to jaundiced patients,hospital admission,traveling and camping.The multivariate regression analysis revealed increased seroprevalence with the low level of outdoor food intake at the different education grading in the absence and presence of food borne diseases.The stepwise logistic regression analysis revealed increase in the seroprevalence of HAV antibodies with increase in the age and the low level of outdoor food intake.

Abstract:Strictures of the biliary system may be benign or malignant in origin.Benign biliary strictures typically occur as a result of surgery-induced injury.Endoscopic treatment for dominant strictures includes catheter dilatation,balloon dilatation,and insertion of stent or nasobiliary tubes.The most common site was the distal (490retrospectively and 52prospectively). Abstract:This study aimed at evaluating the role of grey scale histogram and power Doppler histogram in the diagnosis of different solid focal hepatic lesions.It was found that the mean grey value (MGV)was highest in cases of haemangioma.There was a significant correlation between MGV and the echogenic pattern of focal hepatic lesions.The mean value of vascular indices (VI,FI,VFI)was highest in HCC cases followed by haemangiomas.The mean value of (VI,FI,VFI)was higher in HCC and metastases than in liver cirrhosis.

Abstract:Hepatitis C virus now affects more than 170 million patients worldwide and is a leading cause of chronic hepatitis,cirrhosis and hepatocellular carcinoma.The treatment of choice for chronic hepatitis C is the combination of interferon and ribavirin.Peginterferon has sustained absorption,slower rate of clearance and a longer half-life than unmodified interferon.Determination of hepatitis C virus genotypes is important for prediction of the clinical course and the outcome of antiviral therapy,and this study revealed that genotype 4 was the predominant type (86)among the studied group of Egyptian patients with chronic hepatitis C.In this study,the parameters studied namely;age,weight,liver enzymes level,iron and ferritin and cholesterol levels,viral load,and quasispecies were of no significance as predictors of response to Peg-IFN combination therapy.This study revealed that females and genotype 4a patients respond more favourably to treatment.It also revealed that fibrosis score of the liver biopsy is a good pre-treatment predictor of unfavourable response to treatment.The number of quasispecies did not predict a response to IFN therapy,but it was significantly affected by it.

Abstract:The aim of this study was to determine the seroprevalence of hepatitis B virus and hepatitis C virus among medical students.The results of this study revealed that the overall infection rate for hepatitis B was (14), with the carrier rate was (1.4). Eighty nine (25.4)gave history of full dose HBV vaccine and (82.8)of the non vaccinated group had no markers for hepatitis B and are at risk of infection.Anti-HCV was found in (2.3). Exposure to blood/blood products,mucocutaneous exposure and contact to jaundiced patients were the most common risk factors in our studied group.The overall frequency of anti-HAV IgG was (98.3).

Abstract:This study was conducted on the first 53 cases of adult to adult LDLT in Dar El Fouad hospital.The aim of the study was to investigate the incidence and morbidity of HCV infection recurrence post-LDLT in Egyptian patients and to determine the impact of graft volume on outcome.Recurrence of HCV was universal in terms of viremia.Clinical recurrence of HCV defined by evidence of viral replication by PCR,elevated transaminases,and confirmatory histology,was seen in 26.3of recipients.Clinically recurrent HCV was mild.There was a significant correlation between mean actual graft weight and development of clinically recurrent HCV.There was a significant correlation between mean estimated graft volume and development of clinically recurrent HCV.There was a significant impact of the actual GRWR (1 , 1)on the development of clinical HCV recurrence.Grafts with a GRWR of 1showed a higher incidence of clinical HCV recurrence than those with a GRWR of 1There was a significant directly proportional correlation between estimated graft volume and the time of clinical recurrence of HCV.There was also a significant directly proportional correlation between actual graft weight and the time of clinical recurrence of HCV posttransplantation

Abstract:Background : The development of hepatocellular carcinoma is a multistep process and the result of an accumulation of risks.Many factors may therefore contribute to the final common pathway of HCC.Our aim was to determine the risk factors for the development of hepatocellular carcinoma so that we will be able in future to put a strategy to prevent HCC in Egypt.patients and methods : This study was conducted on sixty cases of HCC as well as thirty patients with liver cirrhosis and another thirty healthy subjects without evidence of chronic liver disease.They were clinically assessed and investigated (Laboratory including complete blood count,liver biochemical profile,viral markers,alpha fetoprotein,anti Schistosomal antibody and serum aflatoxin,and imaging that was mainly by abdominal ultrasonography). results:HCV had a high prevalence among HCC cases while HBV had a declining role.aflatoxin showed a significant role in pathogenesis of this malignancy.HCC commonly presented in males,farmers,heavy smokers and was associated with high incidence of unsafe water supply and exposure to pesticides.The majority of HCC cases (65)had a single major risk factor,and few cases (8.3)developed HCC on top of non cirrhotic liver.conclusion:HCC is multifactorial with interaction of major risk factors like HCV and aflatoxin,minor risk factors like exposure to pesticides and heavy smoking in pathogenesis of this malignancy.HBV infection,which is one of the major risk factors,was found to have a declining role in hepatocarcinogenesis in this work.Also,HCC may occur on top of non cirrhotic liver certainly with viral hepatitis.

Abstract:The aim of this work was to study the changes in serum levels of growth hormone,insulinlike growth factor 1 and its binding proteins 1and 3 in patients with liver cirrhosis,ascites,and hepatorenal syndrome,and the possible relations of these changes to the degree of liver affection,sodium retention and ascites formation.a significant positive correlation was detected between Child-Pugh score and IGF-1-IGFBP index in all cirrhotic patients.while no significant correlation was detected between fractional excretion of sodium,as a representative of sodium excretion,and this index in the same group of cirrhotic patients.our study confirms the previously described changes in the levels of GH,IGF-1 and IGFBP-1 and 3,and their relations to the liver condition.however we could not confirm the relation of these changes to sodium retention and ascites formation.

Abstract:Argon plasma coagulation (APC)is a device intended for thermal coagulation of tissue and has been used successfully in the endoscopic haemostasis of surface bleeding.The aim of this work is the endoscopic and pathologic evaluation of the effect of APC on PHG.This study included 30 patients,10 of them had mild PHG and the other 20 had severe PHG and underwent APC.Its effect was evaluated within 1-4 weeks,endoscopically and histologically.The 20 patients who underwent APC showed endoscopic improvement while 16 of them showed histological improvement.There were no complications among the patients.In conclusion,APC can be recommended as a safe and effective measure for treatment of severe PHG.

Abstract:The current study work studies SOLUBLE IL-2 RECEPTORS,SOLULBE IL-6 RECEPTORS,S FAS and the p53 levels in the fifty four patients IBD and forty seven cancer colon cases also ten normal control subjects,these patients were presented to the endoscopy unit Kaser Al Enin Hospital,Cairo University.There was a significant direct relationship between soluble IL-2 receptors and the colonic extension and the severity of colonic lesions on colonoscopic examinations in IBD cases and the severity of the pathological lesions when examined histopathological.High (although insignificant)levels of p53 were found in seven ulcerative colitis patients with dysplasia in histopathological examination.

Abstract:This study was conducted to determine the clinical and the prognostic degree of COX-2 expression in the liver cells of patients with chronic liver disease and hepatocellular carcinoma.A significant upregulation of COX-2 expression was noted in 47,80 and in 89 of patients with chronic hepatitis,cirrhosis and hepatocellular carcinoma respectively.Failure to responded to antiviral therapy was noted in 57 of patients with COX-2 expression,compared to 44 in patients with negative COX-2 expression.A significant correlation was noticed between hepatocellular COX-2 expression and fibrosis stage.

Abstract:What may be done to diminish the toll of bacterial infections in these highly susceptible patients? In cirrhotic patients,same-day treatment out-clinics and centers should be used instead of hospitalization;if hospitalization is necessary,intensive care units and hospital days should be used in a miserly manner.In hospital,skin cleanliness and universal precautions to diminish patient-to-patient colonization must be observed.Urinary catheter and CVP Line should be avoided whenever possible,and when these are necessary,aseptic precautions should be used to minimize infection.Prophylactic antibiotics with norfloxacin or related agents have been proven effective in all hospitalized cirrhotics with gastrointestinal bleeding (especially chid C and rebleeding)and those with low total ascitic fluid protein or previous episode of spontaneous peritonitis,knowing that 44.7 of all cirrhotic patients will become infected during hospitalization and at least 21 of infected patients will die.however,emperic use of quinolones is not indicated in any infection in cirrhotic patients to avoid infection quinolone resistant It is advisable to culture the ascitic fluid in blood culture bottles as their sensitivity is greater than that of ordinary culture to detect bacterial growth in neutrocytic samples.

Abstract:Hepatocellular carcinoma,is one of the most common malignancies in the world,it is a multifocal and aggressive tumor and it is the fourth most common tumor in the world today.It is now a rather common malignancy in Egypt.HCC is one of the major causes of death because of its high frequency and poor prognosis,it is responsible for about one million deaths annually with only few patients surviving beyond one year.It is one of the few human cancers for which an etiological factor can be identified in most cases.The two main etiological factors for hepotocellular carcinoma are cirrhosis and viral hepatitis (HCV

 HBV), also the risk of HCC increased with prolonged dietary consumption of aflatoxins,alcohol or iron overload.In Egypt the incidence of HCC is increased due to the high prevalence rate of HCV which accounts for most of cirrhosis and HCC cases.Early detection of patients with HCC is attractive because it gives better prognosis,and early detection is not difficult if tumor markers and medical imaging were possible,so the need for easy reliable tumor marker is mandatory because AFP may be within normal in HCC cases,and if elevated it needs high cutoff values to be sure of the diagnosis also imaging is important to be combined with the tumor markers to be sure of the diagnosis,so 3D US used with 2D US to be more accurate,and it can measure the actual volume of the tumor and any local spread.The aim of this study was to determine the value of SACE and Angiotensin II as well as CA 19-9 as possible tumor markers of HCC developing on top of liver cirrhosis.The study was conducted on 40 HCC cases,40 LC cases and 25 normal subjects.We determined the levels of AFP,CA 19-9,SACE and Ang.II for all cases together with liver biochemical profile,viral marker,abdominal US (2D US

 3D US)and histopathologic examination of the hepatic focal lesion in HCC cases.We concluded that the diagnostic sensitivity of AFP at a cutoff 200ng/ml was17.5 and the specificity 100.At a cutoff value of 31ng/ml CA 19-19 showed a sensitivity of 97.5 and a specificity of 90.Serum ACE was 90 sensitive and 82.5 specific at a cutoff 70 U/L while Ang.II was 100 sensitive and 95 specific at 48 pg/ml.Serum ACE showed significant negative correlation with the tumor volume measured by 3D US (r = -0.497,p 0.05).

Abstract:Dual infection by HBV and HCV is of clinical relevance because it may be associated with severe forms of liver disease.This work is designed to study the impact of previous HBV infection (anti-HBc positivity)on chronic hepatitis C patients and to investigate for the prevalence of occult HBV in chronic hepatitis C patients with negative HBsAg.The prevalence of anti-HBc in hepatitis C patients is found to be high and the seropositivity of anti-HBc is often associated with severe liver fibrosis (even in the absence of any evidence for occult HBV DNA)but it does not affect the response to combined interferon-ribavirin therapy.However pegylated interferon based therapy was more effective than standard interferon in chronic hepatitis C patients with anti-HBc and anti-HBs seropositivity.
